I always thought -- even back in S1 -- that the use of "sir" was simply a way for Ianto to maintain distinctions inside his head, to stop himself from thinking of Jack too personally (bad grammar, but you get what I mean, right?). To me, it made sense that the suits and the manners would be an extension of that.
